Healthy Indiana Plan
A new health insurance option for uninsured Hoosiers
In order to provide more affordable health insurance to those in need in Indiana, the State of Indiana and Governor Mitch Daniels have developed a new program called the Healthy Indiana Plan (HIP), a new insurance option for uninsured Hoosiers.
HIP will provide a $1,100 Personal Wellness Responsibility (POWER) account to each adult enrolled in the plan. A basic commercial benefit plan will cover medical expenses that exceed the amount provided by the POWER account. In addition, each participant will receive preventative health services.
Who is eligible?
To be eligible for the Healthy Indiana Plan, you must meet the following requirements:
- Age 19-64
- Annual household income up to 200 percent of the Federal Poverty Level - Click here to view current income levels.
- Not eligible for Hoosier Healthwise or Medicaid
- Not pregnant (pregnancy covered by Hoosier Healthwise)
- Ineligible for health insurance through your employer
- Uninsured for at least six months
How do I apply?
1. Determine your eligibility using the above criteria
2. Contact CHS financial counselors at 765.281.6672, 765.281.6675, or 765.281.6678 or download and print an application from hip.in.gov
3. Complete all sections of the application in blue or black ink.
4. Sign the application where indicated.
5. Gather copies of the materials listed on page one of the Information to Get You Started handout (included with application). These materials must accompany your application.
6. Submit the application to the Healthy Indiana Plan. You can submit the application in any of the following ways:

7. Cooperate with the Healthy Indiana Plan on requests for further information or interviews. HIP will contact you by phone or U.S. Mail if additional information or documentation is needed for your application. HIP requests that you respond quickly to any requests for additional information so your application can continue through the process.
